A belated thank you and greeting. I did print the form you showed me and really just need to find a way to print it in black and white to save on printer cartridges. Or perhaps just copy it and make my own. I like HO 211 primarily because you can work directly from your DR position and because the library is so much smaller. I've purchased a book on HO 208 and will take a look at that also.
