As long as the axis of your level is verticle then it doesn't matter about the axis or the telescope itself. This is how we do do it in the field artillery. The quadrant that is used to measure the elevation is mounted so that it rotates around an axis alligned with the bore of the gun. Then you "cross-level" it and then the quadrant is verticle and provides an accurate measure of the elevation.
