There's been a great variety of approaches to clearing lunars over the long history of the process. Forms have been published occasionally, but they're not particularly successful in the real world of navigation. In the collection at Mystic Seaport Museum, there's a whole book of blank forms for clearing lunars that would have been sold as an add-on for "Norie's Navigator" 175 years ago. They're nice forms, yes, but think about it... blank forms preserved in a museum! That tells us something. It tells us that the forms weren't used. :)
If I can pay my bills this month, I'll try to write up a new "2018" version of "Easy Lunars" and you'll see that lunars are not particularly difficult and do not benefit much from "forms".
