Folks,
Enough "me too" posts on this topic. Sean Johnston, who has suggested that he may be able to produce some copies for sale, still has to figure out his costs and schedule and all that, and when he does, I am sure he will provide contact details so that you all can send in orders. But he is not "taking orders" and Navlist is not a marketplace for placing orders. So no more "me too" messages.
