Terry,
You have the right idea. The v value comes from the daily page, and is used to get the v correction from the Increments and Corrections.
Take a look at the Sight Reduction & Fix tool in the Celestial Tools program (Windows) to see how these are used on a form similar to that of the United States Power Squadrons. Attached is a screen shot of a made-up sight illustrating this.
