A very belated thank you to David Pike. I submited this puzzle in April 2016. There was one reply - from David. Then 5 months later, more replies surfaced, including a definitive one from David - the result of careful research.
I don't regularly follow NavList messages, so I completely missed the resurgence till I stumbled across it by accident today.
I'm just delighted to have this puzzle resolved.
