You could also look on page VIII of the instructions and advice in my Tables for Clearing. A little table inserted there shows errors resulting from 25 different combinations of misalignment and angle measured. Unless I'm overlooking something, the problem of an index mirror out of perpendicular with the plane of the arc is the same as that of line of sight out of parallel. If so, the table should agree with what you get with Alex's formula.
