Maybe its a bit like the Christian cross - the problem as a symbol of the solution. 'This "fouled" anchor is a nautical symbol of distress, signifying the sick and disabled seamen that PHS was created to serve.' (American) Public Health Service http://www.usphsengineers.org/handbook/Chapter2.pdf# Harbourmasters also use a version of this symbol; signifying the safe haven their facilities offer against the threat of distress. An example of this is the (English) Channel port of Kent whose (obsolete) civic insignia inluded: "... a Wreath of the Colours in front of a Foul Anchor erect two Swords in saltire point ... they each support a trident, another nautical symbol..."
