Well, I found it and another one amongst my books.
I had forgotten about the second. It is by far the better one:
It has actually 2 tables, one with 5 digits, the other one with 4 digits.
The last one covers just 6 pages. I have attached a copy of the latter
plus 2 pages of intro.
