Not as accurate as Andres' program (e.g. HP is 59.4' rather than 59.3', etc.), but here are two screen shots from Celestial Tools. In both, I am assuming, as Gary did, that you meant the index correction to be -0.1'.
Look at the ALTITUDE box. In the "SR form" one, it shows the corrections as extracted from the Almanac moon correction tables (main, additional), to the same precision as the Almanac. In the "Parameters" one, it shows the refraction, parallax, semi-diameter, and augmentation to three decimal places, with the final value to one decimal place. No, it is not perfect, but it should give you an idea of what is happening.
