Fully agree here. Since you mention my name in your post, I am simply indicating that I am using Cylinder / Sphere intersection only to determine a first "start up" Approximate position.
Then from this Approximate position I am using Ellipsoid (currently sea level, but no problem for entering altitudes), and refraction for numerical iterations.
For occultations, Ellipsoid is to be used indeed since Lady Moon's parallax is not negligible at all here.
