You also had interest in computer-generated CLS forms. First, they are not permitted for USPS sight folders - you have to use the provided paper form. But, for any other purpose, the CLS Plot function of the program Plottool, available at
http://www.usps.org/national/eddept/n/tools.htm#plottool
does a pretty good job. The only thing that is different is that the longitude scale does not come out at the very bottom of the form, but at the bottom latitude line, as explained in the Plottool 2.0 Manual. (You may only need version 1.5, but the 2.0 manual works for 1.5.) The CLS Plot function does a lot of other tricks you might find handy.
