There are several errors in your example on page 45 and I have attached two computations one with the Bygrave and one with a calculator.
At the beginning you exhanged starting latitude with destination latitude The your calculation of the Az and Hc went wrong somewhere.
I notice that you use my notation for 90 degrees as 89-60 to make subtraction easier but you might like to mention that as an introduction.
That is the only part of your manual that i have had the chance to look at.
