It's my understanding that the Navy stated outright that these incidents were not the result of hacking. Meaning that there was a training deficiency or an lax environment that led to these incidents, resulting in needless deaths, and millions of dollars in damages to two US warships, further more 6 or 7 Naval Personnel have been relieved, and countless others reprimanded. I think this far from over.
There can be a combination of many root causes and I think they are still ironing it all out.

Relaxed environment
Complacency
High Operations Tempo
Burn out of personnel
Poor Command Leadership
