This is the DM 17 or Great Circle Sailing Chart as offered at Maryland Nautical for $28.00 incl. They are a print on demand and mail order Nautical Chandlery in Locust Point, Baltimore.

look at mdnautical dot com and do a search on the website for "great cirle sailing chart" instead of DM17 or a variation.
