As an example of a malfunctioning digital navigation system using maps, there's Google Maps, which is getting progressively worse.
This summer, while on vacation on the island of Malta, Google Maps kept telling me to take the roundabout, but there wasn't one. This happened several times.
Good old paper nautical charts :)
